Completion of Forms

Please allow 7 days for any school, camp, college and etc., to be completed by our office. We receive many forms daily and each chart must be reviewed in order to complete them.

Be aware of the following:

1. Complete/ sign any sections required by “parent/patient”. We will not complete the form without it being done.
2. Your child must have had a physical in the last 12 months or we will not complete the form.
3. We do NOT fax forms
4. We will mail the form only with an addressed envelope from you attached to the form
5. If you have access thru our patient portal, “Gateway” you can print off your child’s routine physical form. Don’t have “Gateway”?   Ask how to enroll!
