Your Child's First Visit
If you are pregnant, we would be happy to provide care for your new baby. A
pre-natal visit can be scheduled if you would like to visit the office and meet
with a pediatrician before your baby is born. There is no charge for this visit.
We would like to see your new baby in the office within 2-3 days after leaving
the hospital. No routine immunizations or labwork are needed at this visit
however the provider might order a bilirubin test if there are any signs of
jaundice. We will address any of your concerns regarding the baby’s health as
well as routine newborn care. Please call the office as soon as you leave the
hospital to schedule this appointment.
If you already have a child and are transferring his/her care to Middleboro
Pediatrics, we require a copy of your child's medical records from his/her
previous pediatrician. You will need to contact their office in writing to make
a formal request to transfer the records. You can use our
Transfer
of Records form. You also need to contact your health insurer and
request to have your child's primary care doctor changed to one of the doctors
at Middleboro Pediatrics.
We will need some information from you at your child’s first visit. You can
streamline the process by printing out the following forms and bringing them
with you to the office:
