We are now offering our patients a new benefit called "Webview." Webview will
allow you to access parts of the medical chart online through a medically
secure, HIPAA-compliant website. Features of the program will enable you to do
the following from your home computer:
Look up laboratory test and radiology results
Print out completed forms (e.g. physical forms, immunization records,
school medication forms)
Look up growth chart data
Look up previously scheduled appointment dates
Look up current and previous prescription information
E-mail the office for concerns such as referral requests, medication refill
requests, billing or insurance questions
E-mail a physician or nurse practitioner any NON-URGENT medical questions
It is important to remember that e-mail can only be used for non-urgent
questions, as we are unable to guarantee that it will be read the same day. IF
YOUR CHILD IS SICK AND YOU REQUIRE AN IMMEDIATE RESPONSE, WE RECOMMEND THAT YOU
CALL OUR OFFICE DIRECTLY.
ENROLLMENT
For children ages birth - 12 years, parent/guardians may either fax or
deliver the completed form in person to our office.
For children ages 13 years - 17 years, the form must be completed in person
in our office. We require witnessed and signed consent by the adolescent patient
to allow a parent or guardian access to Webview. This requirement is due to laws
relating to an adolescent’s right to seek certain aspects of medical care
confidentially.
Patients age 18 years and older will have their own access to Webview and can
either fax or deliver the completed form in person to our office.

form. You will need one form for each patient.
When you sign up for Webview, you will be assigned a temporary password:

The first time you login, you will need to use this password. You will then be
required to change the password to one of your choosing.
